Introduction: Ankyloglossia may impair breastfeeding by limiting tongue mobility and interfering with the mechanics of sucking, thereby contributing to early weaning. Objective: To assess the prevalence and associated factors of early weaning among Amazonian infants undergoing frenotomy, as well as to develop educational technologies aimed at promoting breastfeeding. Method: A prospective cohort study with a quantitative approach was conducted at Dental Specialty Centers in Manaus, Amazonas. The sample consisted of 137 mother–infant dyads, followed on the day of surgery and at 30 and 180 days post-procedure. Sociodemographic, healthcare, and clinical variables related to breastfeeding were analyzed using descriptive and inferential statistics. Results: In the pre-intervention period, a high frequency of breastfeeding difficulties was observed, including nipple pain, frequent choking, and clicking sounds during feeding. After frenotomy, there was a statistically significant improvement in all evaluated parameters. The prevalence of early weaning was 10.2%, and a higher number of prenatal visits was associated with a lower prevalence of this outcome (PR = 0.33; 95% CI: 0.12–0.89). Conclusion: Frenotomy contributes to the improvement of breastfeeding-related symptoms and may support the maintenance of breastfeeding. Adequate prenatal care and the use of educational technologies are relevant strategies for preventing early weaning.
