Introduction: Patient Handoff consists of the transfer of information, therapeutic planning and responsibility for patient care from one health professional to another, between work shifts. Objectives: Improve practice compliance with best evidence in the patient handoff process among nurses in an Intensive Care Unit (ICU) and develop and validate the content of a patient handoff tool. Method: Evidence implementation study according to the JBI method and methodological study to validate the content of the handover tool, according to the Pasquali method. Carried out in the ICU of the Hospital e Pronto Socorro Dr. João Lúcio Pereira Machado (HPSJL), in Manaus, Amazonas. Data were collected from May 27 to August 23, 2024 and nurses from HPSJL and expert judges participated in the study in the validation phase. The data was analysed using absolute and relative frequencies and to validate the patient handoff tool content we used the Content Validity Index (CVI). The project was approved by the Ethics and Research Committee. Results: The baseline audit showed low compliance between practice and best evidence, with four of the 6 audited criteria showing no compliance and compliance in two ranging from 20 to 65%. After implementing the technical and technological product and educational intervention, all items showed an increase in compliance with best practices, ranging from 90 to 100%. The instrument developed presented an average overall CVI of 92%, with 96% for clarity, 94% for relevance and 86% relevance. Conclusion: As a technical and technological production, a patient handoff tool was developed for nurses in the ICU, focusing on relevant aspects of Nursing practice and special attention to vulnerable conditions, such as the homeless population. The perspective is that the developed product will contribute to the sustainability of the best practices implemented, effective communication, patient safety and prevention of adverse events.
