Aplicativo para manejo clínico dos pacientes vítimas de hemorragia subaracnoidea por aneurisma cerebral no Hospital Geral de Roraima

Resumo

Justification: Patients who suffer from subarachnoid hemorrhage (SAH) account for 5% of all cerebrovascular accidents. However, SAH alone is responsible for 25% of the total deaths related to cerebrovascular accidents. The lack of guidelines in managing these patients, from hospital admission to their stay in the Intensive Care Unit (ICU), leads to varied and inconsistent outcomes, increasing mortality rates, length of hospital stay, and functional dependence after discharge (for those who survive). General Objective: Assess the current panorama of clinical approaches to patients with subarachnoid hemorrhage caused by cerebral aneurysm at the General Hospital of Roraima, aiming to create an application containing a protocol of procedures based on the latest recommendations. Specific Objectives: Analyze the current practices of medical professionals involved in the care of SAH patients in the ICU; to standardize the assessments of patients with spontaneous SAH and establish uniform targets for vital sign measurements; to structure and standardize the treatment of complications arising from SAH. Method: A qualitative, descriptive, exploratory study using the Design-Thinking methodology, based on the double diamond model. Results: There was significant heterogeneity in the practices of professionals responsible for the care of neurocritical patients in the ICU, often contrary to the recommendations of the American Heart Association (AHA) Guideline. The application, to be made available on the iOS platform, includes a decision tree based on the 2023 AHA Guideline, aiming to standardize the clinical management of these patients. Conclusion: Standardizing the treatment of SAH patients is essential to ensure better clinical outcomes. The expertise of neurointensivists is crucial in organizing flowcharts and periodically reassessing patients. The proposed application will be a powerful tool in the hands of doctors working in this sector, providing precise information for diagnosis and swift management within the hospital.
